Each plan outlines small, measurable steps that move toward larger life goals, whether that’s maintaining stable housing, finding a job, or reconnecting with family. Whenever appropriate, we involve family members, case managers, and community supports, making sure the client has a team around them to reinforce progress. Plans are reviewed regularly, so goals stay meaningful and clients can celebrate each success.
